Summary Description : We have an exciting opportunity for a full-time Licensed Family Specialist in our Assertive Community Treatment (ACT) program. The ACT team provides non-traditional community-based mental health treatment to promote the recovery and community integration of 68 adults with serious mental illnesses and co-occurring substance use disorders. The specialist delivers routine treatment services, integrates family goals and services, and provides family psycho-education. The Family Specialist supports consumers to achieve wellness, recovery, and positive relationships in the community, utilizing evidence-based practices and a trauma-informed and person-centered approach.
Key Responsibilities :
- Conduct comprehensive assessments of psychiatric history, mental status, diagnosis, physical and dental health, substance use, education, employment, social development, functioning, daily living activities, and family relationships.
- Address housing, income support, education, vocational training, social supports, employment, and primary care needs.
- Involve families and social supports in the consumer’s treatment with the team.
- Provide engagement, problem-solving, wellness self-management, medication support, family support and treatment, individual mental health, relapse prevention, harm reduction, and substance use counseling.
- Provide 24-hour crisis intervention on a rotating basis.
- Deliver 80 percent of treatment contacts in the community.
- Document client progress according to established methods and procedures.
- Participate in daily morning team meetings and treatment planning review meetings.
- Follow ACT policies & procedures, agency policies, and maintain professional standards.
- Use client data for clinical decision-making, quality improvement, and tracking progress.
- Commitment to cultural competency, diversity, and inclusion.
- Share client data with clients to facilitate shared decision-making.
- Other duties as assigned
