What are the responsibilities and job description for the Community Liaison (Hospice) position at Greater Valley Hospice?
Summary: The Community Liaison serves as a compassionate and knowledgeable representative of Greater Valley Hospice, building and maintaining positive relationships with key stakeholders in the community, including healthcare providers, community organizations, and contracted facilities.
Key Responsibilities:
*Develop and implement community awareness and education utilizing to increase awareness of hospice services and the benefits of hospice care.
* Represent the hospice at community events, meetings, and conferences.
* Build and maintain community building relationships with home health, provider services and assisted livings.
* Collaborate with community organizations, such as senior centers, support groups, and faith-based organizations.
* Provide education and resources about hospice care to the community.
* Identify and respond to community needs related to end-of-life care education.
* Serve as a liaison between the hospice team and the community, facilitating communication and collaboration.
Qualifications:
* Healthcare experience preferred.
* Excellent interpersonal and communication skills, with a focus on compassion and empathy.
* Strong community engagement and relationship-building skills.
* Knowledge of the local community and its healthcare resources.
* Ability to work independently and as part of a team.
* Task orientated and time management skills required
* Passion for hospice care and Greater Valley Hospice's mission and values.
Job Type: Full-time
Pay: $50,000.00 - $55,000.00 per year
