*Job Overview*

We are seeking an Intensive In Home Team Lead to join our team. The ideal candidate will have experience in home community care, medical documentation, and conflict management.

Licensed in the State of North Carolina, as a physician, licensed psychologist, licensed clinical social worker, licensed clinical mental health, licensed clinical social worker associate, licensed clinical mental health counselor associate.

licensed marriage and family therapist, licensed clinical addictions specialist, certified clinical nurse specialist in psychiatric mental health advanced practice, or a certified nurse practitioner in psychiatric mental health advanced practice. An associate level professional actively seeking licensure may serve as the team leader conditional upon being fully licensed within 30 months from the date of hire.

 Minimum 1 year of experience with the children and adolescent population with

behavioral needs or a substance use disorder

 Criminal background check present

 Not listed on the North Carolina Health Care Abuse Registry

 Not listed on the exclusion database with the Office of Inspector General

 No IIH team member who is actively fulfilling an IIH team role may contribute to the

staffing ratio required for another service during that time

 Knowledge of federal and state laws and regulations

 Strong computer skills

 Must demonstrate integrity, sound judgment, demonstrated leadership skills,

organizational and strong interpersonal skills.

 Experience working with disadvantaged populations, helpful and knowledge of mental

 Excellent oral and written communication skills needed, strong organizational ability

 Outstanding skills in data collection, analysis, and presentation. Proficient in Microsoft Word and Excel

*Skills*

Maintain a caseload ratio of 1 to 12, ratio shall not exceed

 Must be available 24 hours per day, 365 days per year, to include crisis intervention

 Provide individual and family therapy for each beneficiary served by the team

 Designate the appropriate team staff such that specialized clinical expertise is applied as clinically indicated for each client

 Provide and coordinate the assessment and reassessment of the client’s clinical needs

 Provide clinical expertise and guidance to the IIH team members in the team’s

interventions with the client

 Provide the clinical supervision of all members of the team for the provision of this

service. An individual supervision plan is required for all IIH team members exclusive of

the Team Leader

 Coordinates and oversees the initial and ongoing assessment activities

 Convene the Child and Family Team for person-centered planning

 Consult with identified medical (for example, primary care and psychiatric) and non-

medical (for example, DSS, school, DJJDP) providers, engaging community and natural

supports, and including their input in the person-centered planning process, as needed

 Ensure linkage to the most clinically appropriate and effective services including

arranging for psychological and psychiatric evaluations, as needed

 Provide and coordinate behavioral health services and other interventions for the client or other family members with other licensed professionals and Child and Family Team members, as needed

 Monitor and document the status of the client’s progress and the effectiveness of the strategies and interventions outlined in the PCP, as needed

 Ensure timely and accurate documentation in accordance with state and agency

requirements.

 Conduct regular reviews of clinical records and service delivery

 Attends conferences, meetings, audits, and training sessions as requested

 Comply with Health Insurance Portability and Accountability Act (HIPAA) and adhere to and follow the organizations policies and procedures

 Accepts other job duties as delegated

Job Type: Full-time

Pay: From $35.00 per hour

Expected hours: 40 per week
