Position Summary: The Clinical and Wound Care LVN provides comprehensive nursing care with a focus on wound management in both clinical and home care settings. This role requires expertise in assessing, treating, and monitoring chronic and acute wounds, ensuring high standards of care for patients, and collaborating with healthcare professionals to improve patient outcomes.
Key Responsibilities:

Wound Care Management:
- Assess and evaluate patients' wounds, including identifying the type, size, and condition of the wound.
- Develop and implement individualized wound care plans, utilizing appropriate dressings, topical treatments, and other interventions.
- Perform regular wound debridement (as appropriate) and apply dressing changes.
- Monitor progress of wound healing and report any signs of infection or complications to the supervising physician or healthcare provider.
- Educate patients and families on proper wound care techniques, hygiene, and lifestyle modifications to support healing.

Patient Assessment:
- Conduct comprehensive nursing assessments, including medical history, vital signs, and physical condition.
- Collect and document patient data relevant to wound care, such as wound measurements, exudate levels, and surrounding tissue condition.
- Monitor patients for changes in condition and provide necessary updates to the medical team.

Collaboration and Communication:
- Work closely with physicians, wound care specialists, and other healthcare team members to develop treatment plans and coordinate patient care.
- Maintain clear and accurate patient records, ensuring all treatments, observations, and patient responses are documented according to healthcare protocols.
- Communicate effectively with patients and their families, offering emotional support and education on managing wounds and promoting healing.

Clinical Duties:
- Administer prescribed medications, injections, and treatments as ordered by physicians.
- Provide general nursing care to patients, including monitoring vital signs, administering IV fluids, and assisting with daily activities as needed.
- Assist in the development of care plans and participate in multidisciplinary team meetings to review patient progress and treatment adjustments.

Infection Control and Safety:
- Adhere to infection control protocols to prevent contamination during wound care procedures.
- Ensure that the clinical environment is safe and well-maintained for patients and staff.

Patient Education:
- Provide wound care education and support to patients, families, and caregivers to ensure proper follow-up care at home.
- Instruct on the signs and symptoms of infection, when to seek medical care, and preventive measures for wound management.
Qualifications:
- Education: Graduation from an accredited LVN/LPN program.
- Licensure: Valid LVN (Licensed Vocational Nurse) license in the state of employment.
- Experience: Minimum of 1-2 years of clinical nursing experience, with specialized experience in wound care preferred.
- Certifications: Wound care certification (e.g., Wound Care Certification (WCC) or Certified Wound Specialist (CWS)) is preferred but not required.
