Responsibilities
- Conduct assessments to identify client needs and develop individualized service plans.
- Coordinate access to resources.
- Provide/Coordinate intervention and management support to clients facing immediate challenges.
- Supervise and mentor clients in navigating social services and community resources.
- Collaborate with multidisciplinary teams to ensure comprehensive care for clients.
- Maintain accurate documentation of client interactions, progress, and outcomes.
- Advocate for clients’ needs within the community.
- Stay informed about local resources and relevant services.
Requirements
- Associates or Bachelor’s degree in Social Work or a related field
- Experience in social services is prefered.
- Strong interpersonal skills with the ability to build rapport with diverse populations.
- Excellent organizational skills and attention to detail in documentation and reporting.
- Ability to work independently as well as collaboratively within a team environment.
