Essential Functions
The following duties are normal for this position. These are not exclusive or all-inclusive. Other duties may be required and assigned.
· Facilitates nursing care provided to all residents and includes residents in decision making processes;
· Actively engages residents in participation in daily activities which are part of resident’s rhythm of daily life (including desired hygiene, grooming, and mobility);
· Assesses resident’s needs at the time of admission and continually evaluates residents’ condition during the duration of their stay and promotes the highest functional level of independence possible (keeping in compliance with state and federal regulations, and the community’s safety procedures);
· Leads and directs care partners by providing support, encouragement, and direction as appropriate;
· Makes frequent rounds and utilizes the nursing process to identify resident conditions that require further evaluation and treatment by a physician;
· Reports significant medical changes to the relevant Team Members, Physician, DON, HSA and designated family members as appropriate;
· Responds to emergency situations, carries out physician orders, and documents the event, interventions, and outcomes;
· Accurately and completely performs direct nursing services, such as, medication administration, treatments, personal care and other nursing procedures.
· Accurately receives documents and transcribes Physician Orders with the appropriate follow-through in a timely fashion, specifically advocating resident safety;
· Assures proper documentation including but not limited to: consents, summary notes, care plans, medication records, etc.
· Reports and documents vital information on all guests’ paperwork;
· Actively engages and participates in the community’s QAPI program;
· Completes risk management, progress notes, assessments, weekly summaries, and care plans;
· Coordinates the residents’ plan of care with the interdisciplinary team (IDT), physicians, medical personnel, and resident’s family/responsible party;
· Interacts with other departments such as laboratory, radiology, dietary, facilities, information technology, pharmacy, social services, activities, etc;
· Collects lab specimen for testing, obtains and assesses vital signs, weights, I/O’s, etc;
· Makes necessary documentation corrections as a result of medical record audits;
· Proactively participates in the move-in and move-out procedures for each resident;
· Builds genuine relationships with residents through discussion and review of their history;
· Contributes to creating a home environment in each community
· Checks on operating condition of equipment and notifies appropriate personnel when necessary;
· Provides effective and courteous service to all residents, family members, guests, and team members; is clear in communication and handles conflicts appropriately;
· Assists in Resident and family education;
· Makes daily rounds and reports any unusual changes to the Director of Nursing.
· Updates the Director of Nursing on any administrative concerns, such as family issues and the like.
· Assures that nursing staff is carrying out their respective duties toward resident care, which includes monitoring call lights so that they are promptly answered; and other resident and family care needs are met appropriately and in a timely manner.
· Assists in disciplinary actions.
· Ensures Workers’ Compensation forms are completed when work related accidents occur.
· Handles all admission inquiries when either the Admissions Coordinator or Director of Nursing is not available.
· Takes responsibility to lead the team and ensure that all procedures are followed during emergency situations.
· Acts as Senior Manager on campus during PMs, NOCs and all three shifts on the weekends.
· Ensures that the community Administrator of Health Services and Director of Nursing are notified of emergency situations that occur after hours or on weekends.
· Attends and actively participates in in-service training and meetings as scheduled; incorporates training received into daily work;
· Assists Licensed Nurses and other IDT members as needed;
· Maintains daily records consistent with regulations;
· Promotes and protects the rights of each resident;
· Performs other related essential duties as required;
REQUIREMENTS:
Education:
· Completion of Licensed Registered Nursing Program (RN)
Experience/Training:
· Clinical observation and assessment skills
· Proficient computer skills necessary to complete documentation, review electronic health records and communicate with team members.
· Minimum of 3-5 years of professional managerial experience in related field
Certificates, Licenses, Registrations:
· Current RN license in California
· Current CPR certification
Preferred:
· Bachelors of Science in Nursing
· Senior Care or Rehabilitation Experience
