This unique opportunity is ideal for a talented Physical Therapist looking to lead and inspire a team while providing hands-on patient care. In this role, you’ll spend significant time treating patients and also support our Regional Director with overseeing therapists, ensuring high-quality care across Senior Communities and Skilled Nursing Facilities. As the primary contact for administrators at partner facilities, you will actively market and promote our services to both new and existing partners.

Physical Therapist Responsibilities:
- Conducting initial and ongoing evaluations of residents' physical capabilities, including strength, mobility, and endurance.
- Identifying barriers to independence and functional movement.
- Developing individualized treatment plans to improve residents' mobility, strength, and functional independence.
- Addressing challenges related to mobility, balance, and pain management.
- Assisting in recovery from injuries, surgeries, or illnesses through targeted physical therapy exercises and techniques.
- Helping residents improve their ability to perform activities of daily living, such as walking, dressing, and transferring.
- Implementing strengthening, flexibility, and coordination exercises to enhance motor function and prevent further injury.
- Educating residents and staff on fall prevention techniques and mobility strategies.
- Recommending and training in the use of assistive devices like walkers, wheelchairs, or braces.
- Collaborating with a multidisciplinary team, including nurses, occupational therapists, speech therapists, and other healthcare professionals, to ensure holistic care.
- Participating in care planning meetings to set and review therapeutic goals.
- Maintaining accurate records of assessments, treatments, and progress to ensure compliance with healthcare regulations.

Qualifications
- Graduate of an accredited school with a bachelor’s, master’s, or doctoral degree in Physical Therapy.
- License as required by the State of Wisconsin to practice as a Physical Therapist.
