We are hiring an LPN - LTC for a Permanent role in Kane, PA
Job details:
- Start date: ASAP
- Practice setting: Long-term care
- Full-Time: 36 hours per week
- Shift length: 12 hours
- Shift: Nights - 6:00 PM - 6:00 AM
- Weekend Requirement: Rotating Weekends
- On-call Requirement: As needed
- Holiday Requirement: As needed
- Any Travel Required (between more than 1 Location): None
- COVID-19 Vaccine Requirement: Preferred
Job Requirements:
- Experience: New Grads can apply
- Current state of Pennsylvania license as a LPN.
- Education - Graduation from an accredited nursing program.)
- Certifications - BLS from (AHA)
Job Description/General Duties:
Notify the RN Supervisor immediately of a significant change in the resident’s condition
Reports and records changes in resident condition to the RN Supervisor
Communicates resident transfers to immediate family and via email to department heads
Completes transfer form for all transfers
Updates care plans with clear, concise, time-limited goals to reflect resident condition and directs team to implement approaches
Assists physicians with rounds, examinations, and treatments as needed
Collects laboratory specimens, e.g., urine, cultures, stool, etc., and arranges for delivery to Hospital with accurate and complete requisitions. Documents in PCC when laboratory work is done
Pay and Benefits details:
- Annual Pay: $48,672 - $49,982 per annum (36hr week)
