Position Summary 

Responsible for managing all new intakes starting with the referral and ending with admission decision and case assignment. Must be able to engage patients across the life span. Engage community partners in making referrals. This position is full time in person and requires travel to other community partner locations.

 

Responsibilities

  • Oversee the referral and intake process.
  • Ensure all intake assessments and screenings are completed, and an admission decision is made within 30 days of the referral.
  • Perform crisis intakes and admissions at first contact.
  • Assess the patient’s appropriateness for admission and manage the scheduling of all pre-admission appointments; make referrals for individuals needing different level of care and maintain waitlist.
  • Submit intake availabilities on Zocdoc.
  • For patients on a waitlist for individual counseling, ensure they are enrolled for group counseling, case management and or peer services in the interim.
  • Make initial case assignments based on patient need, and provider skills and availability.
  • Ensure high risk designation, and safety plans are completed as part of admission for applicable patients.
  • Complete reports specific to intake for grants are completed in a timely fashion.
  • Complete all required documentation concurrently in a timely manner in the agencies EHR, including but not limited to patient demographics, emergency contacts, other provider and medications currently taken, assessments, notes, treatment and safety plans.
  • Meet productivity benchmarks and ensure those staff that report to you do the same.
  • Provide supervision to staff assigned to complete intakes and their schedules regarding off site partners and scheduling of intakes during clinic operating hours.
  • Serve as liaison and advocate with other agencies.
  • Outreach in the community and develop referral partnerships.
  • Complete utilization reviews for treatment other than admissions.
  • Serve as a member of the senior leadership team within the clinic.
  • Use time efficiently and personally well organized.
  • Perform other tasks and special projects as assigned by the direct supervisor.

Qualifications

  • Licensed Clinical Social Worker (LCSW) and preferably one to two years’ experience.
  • Strong commitment in ongoing learning and training in the areas of abuse, domestic violence, child welfare, SUD and modalities of treatment.
  • Effective written, verbal, organizational, interpersonal skills and computer literacy required plus knowledge of evidenced-based practices.
  • Must have clinical skills, including ability to assess individuals for proper treatment and evidence of trauma and crisis.
  • Must be able to establish priorities and a course of action for handling multiple tasks.

Vaccination Requirement
