In this exciting role, you’ll play a key part in helping our veteran clients maintain their independence and achieve peace of mind by providing exceptional case management and staffing support.
What You'll Do:
· Client-Caregiver Matchmaking: Ensure clients receive the best care by referring caregivers that best match their specific needs.
· Relationship Management: Build and maintain strong, trusting relationships with both clients and caregivers to ensure satisfaction. Build and maintain strong, trusting relationships with the VA staff.
· Retention & Introductions: Conduct retention visits and introduce clients to their caregivers.
· Assess Client Needs: Perform comprehensive assessments to fully understand each client’s needs, challenges, and objectives.
· On-Call Rotation: Participate in on-call duties on a rotating basis to ensure continuous support for clients.

What We Expect From You:
· Experience & Education: Associate degree in Human Services or a related field preferred (not required). Relevant experience in home care, scheduling, or a similar role is a plus.
· Strong Communication Skills: Excellent verbal and written communication skills, with a pleasant phone manner.
· Tech-savvy: Proficiency in Microsoft Office and other computer programs.
· Organized & Detail-Oriented: Ability to solve problems creatively, stay organized, and maintain attention to detail.
· Passion for Helping Others: A strong desire to improve the lives of others and contribute to the success of the team.
