What You'll Do:
- Client-Caregiver Matchmaking: Ensure clients receive the best care by referring caregivers that best match their specific needs.
- Relationship Management: Build and maintain strong, trusting relationships with both clients and caregivers to ensure satisfaction. Build and maintain strong, trusting relationships with the VA staff.
- Retention & Introductions: Conduct retention visits and introduce clients to their caregivers.
- Assess Client Needs: Perform comprehensive assessments to fully understand each client’s needs, challenges, and objectives.
- On-Call Rotation: Participate in on-call duties on a rotating basis to ensure continuous support for clients.

- Experience & Education: Associate degree in Human Services or a related field preferred (not required). Relevant experience in home care, scheduling, or a similar role is a plus.

- Tech-savvy: Proficiency in Microsoft Office and other computer programs.
- Organized & Detail-Oriented: Ability to solve problems creatively, stay organized, and maintain attention to detail.
