Salary Range:
- RN: $37.25 - $42.25/Hour (Based on Experience)
- LPN: $29.50 - $34.50/Hour
Schedule:
- 12:30pm - 9pm
- Part Time, working 56 hours per pay period.
- Weekend and Holiday Rotation.
- Must be available to work every other weekend.
Position Summary:
Responsible for the assessment/observations care and services provided to assigned residents. Supervises/monitors assigned staff. Responsible for the management of the facility in the absence of the DON and Administrator (LALD). Services are provided in accordance with the mission of Guardian Angels. Remains compliant with State and Federal rules and regulations. Follows established facility policies and procedures. Responsible for work supplies and tools to ensure that resources are used wisely. Supports and implements all safety practices.
Job Duties:
- Completes initial assessment of potential residents. Assessment is completed within two weeks of admission.
- Assures residents maintain admission criteria, reassess and recommend alternative placement if needed.
- Implements plan of care and service agreement with all new residents. Reviews/revises service plan as needed and routinely each year.
- Monitors and maintains resident’s charts and clinical progress notes to assure care plans are accurate and current.
- Communicates to physicians and other interdisciplinary team any change in resident’s condition or change in needs.
- Facilities coordination of resident services with service providers.
- Responsible for ordering/renewal of medications as well as physician pharmacy follow-up.
- Assures all physician orders are signed and implemented within 24 hours.
- Provides weekly medication set-up for administration by unlicensed staff. Assures that procedures and policies are followed for medication administration. Discusses errors and needed corrections with staff.
- Educates and assesses unlicensed staff in medication administration and other medical procedures within licensure guidelines.
- Assures new medication forms are in place by the first of each month. Reviews each resident’s monthly medication form to ensure accuracy.
- Communicates resident information to staff via 24 hour communication book.
- Documents disposal of medication per procedure.
- Supervises medication practices and medical equipment utilized at the facility.
- Performs supervisory visits 14 days after admission and at least every 90 days thereafter.
- Assures that staff clearly understands and follows all resident care policies/procedures.
- Is available for on-call medical questions and emergencies via phone as needed.
- Addresses resident and family concerns.
- To assist in providing a dignified, respectful, resident-centered environment for residents.
Qualifications:
- Current license as an RN or LPN in the state of MN required.
- CPR certification is required.
- Three to Five years prior nursing experience is preferred.
- On-Call rotation is required.
- Excellent verbal and written communication skills.
- Self-motivated, ability to prioritize.
