Guardianship Program of Dade County, Inc. is a leading and unique social services agency that works with a diverse and vulnerable adult population (including young adults 18 years and older) throughout all of Miami-Dade County. We currently have an open position for an Intake Coordinator.
SUMMARY DESCRIPTION:
The Intake Coordinator performs initial evaluations and assessments in order to stabilize and protect the individual client. This person will perform important functions in the early stages once the agency is appointed legal guardian by the courts to intervene and assist.
Good judgement and communication skills are essential as the Intake Coordinator will be working closely with the client, any applicable family members, and other agencies such as Department of Children and Families (DCF) and local law enforcement, as needed.
The Coordinator will generally assist with the implementation of services and, when possible, with reunifications of willing and able family members and/or assisting the individual regain self-independence.
ESSENTIAL DUTIES AND RESPONSIBILITIES:
Reviews all notices of court hearings, legal petitions, and other related reports concerning the client and appointment. Works closely with other agency departments.
Completes initial assessments and client visits at the client’s residence, or residential facilities such as ALFs, or hospitals, and within prescribed timelines, normally 72 hours. Visits may include coordination with other agencies such as DCF’s Adult Protective Services and law enforcement.
Deliverables include completing all statutorily required documentation, such as the initial plan and the intake evaluation/assessment. Investigates and obtains background information and any other pertinent information in order to protect the client and property.
Coordinates within the agency and other agencies and providers for an appropriate placement for the client.
Writes evaluations and time logs for all assigned clients for each pertinent duty performed and updates internal Guardian Management System as required.
QUALIFICATIONS:
The person should have proficiency in Word, Outlook, Excel, and Internet research.
Experience working in a social services environment with adult populations is helpful. A strong commitment for social work is ideal.
Must have a valid Florida driver’s license, reliable transportation, and be capable of unrestricted travel in the performance of duties.
EDUCATION and/or EXPERIENCE:
Bachelor's degree (B.A. or B.S.) from an accredited, four-year college or university, preferably in Social Work or Criminal Justice, or at least three years related experience.
