Job Title: Behavioral Health Care Coordinator (LCSW)

Position Overview:
As a Behavioral Health Care Coordinator, you will serve as a vital link between our facilities, account managers, and the clinical team. Your role is key to streamlining the patient onboarding process and ensuring that our senior residents receive the attentive, coordinated mental health care they deserve. This position is perfect for an LCSW who excels in case management, communication, and patient advocacy within complex care settings.
What You’ll Do:
Liaison & Communication:
- Serve as the primary point of contact between nursing homes, assisted living facilities, account managers, and our clinical staff.
- Facilitate clear, effective communication to support a smooth onboarding process and ongoing patient management, ensuring that the unique needs of our senior residents are met.
Documentation & Compliance:
- Collect insurance face sheets from facilities and secure all necessary documentation, including Power of Attorney (PoA) records.
- Maintain an updated list of PoA contacts and collaborate with facilities and internal teams to complete the Patient Readiness Form—ensuring we have a precise picture of the seniors we will serve.
- Ensure that all documentation is accurate, complete, and compliant with both internal standards and regulatory requirements.
Facility Engagement & Monitoring:
- Visit assigned facilities at least three times per week to check census data, identify new residents, and monitor changes in resident status.
- Provide timely updates on facility census, new admissions, and patient transitions to account managers, billing teams, and therapists—closing the loop on our processes and ensuring our senior patients receive consistent care.
Patient Advocacy & Care Coordination:
- Champion the patient journey from onboarding through ongoing care, advocating for the mental health needs of our senior residents.
- Work hand-in-hand with account managers and clinical staff to address patient-specific concerns and optimize care delivery for seniors.
What We’re Looking For:
- Must hold a current Licensed Clinical Social Worker (LCSW) credential.
- Demonstrated experience in case management, especially within mental health settings.
- Hands-on experience working in nursing homes, assisted living facilities, or similar environments that cater to senior citizens.
- Proven experience with insurance documentation and related processes.
Skills & Personal Attributes:
- Excellent interpersonal and communication skills with the ability to build effective relationships with diverse teams and facility staff.
- Strong organizational skills and a keen eye for detail.
- Ability to work independently, manage multiple responsibilities, and adapt to evolving workflows.
- Willingness and ability to travel to assigned facilities at least three times per week.
