About Guiding Star

Guiding Star Mental Health is on a mission to bring hope and healing to the hearts and minds of senior citizens through compassionate talk therapy. We specialize in serving seniors in nursing homes, assisted living facilities, and other senior living communities. We are expanding and seeking a Case Management Coordinator to help streamline the patient intake and onboarding process for our mental health services.

Position Summary

The Case Management Coordinator is responsible for managing the intake process and coordinating care for new patients. This role ensures all necessary paperwork, insurance verification, and documentation is completed accurately and in a timely manner. You will serve as the liaison between facilities, families, and our internal clinical and operational teams to ensure patients are set up for success.

Responsibilities

Patient Intake & Documentation

  • Complete and manage all patient onboarding forms.
  • Collect and verify documentation such as insurance face sheets, Power of Attorney forms, and consents.
  • Maintain accurate and timely records using our EHR system.
  • Ensure compliance with HIPAA and all regulatory documentation standards.

Facility & Family Communication

  • Serve as the main point of contact for facilities regarding census changes, new admissions, and documentation needs.
  • Conduct outreach to assigned facilities at least three times per week.
  • Communicate with families and responsible parties as needed to complete paperwork and clarify care-related questions.

Coordination & Case Management Support

  • Identify eligible residents and help coordinate services between facility staff, families, and Guiding Star team members.
  • Collaborate with therapists and account managers to ensure patients are prepared for services.
  • Track onboarding progress and follow up on incomplete or missing documentation.

Qualifications

  • 1 years of experience in healthcare intake, case coordination, or healthcare administration
  • Experience working with senior populations or in long-term care settings preferred
  • Strong attention to detail and organizational skills
  • Excellent written and verbal communication skills
  • Ability to manage multiple priorities and work independently
  • Knowledge of HIPAA standards and insurance documentation processes
  • Experience with EHR systems; familiarity with IntakeQ is a plus

Preferred Certifications (Not Required)

  • Certificate in Case Management
  • HIPAA Certification
  • Medical Office Assistant Certificate

Pay: $20.00 - $28.00 per hour

Expected hours: 35 – 40 per week
