PHI Security: Has access to clinical records subject to the limitations as noted in GuidingPoint Policies and Procedures pertaining to “Routine Uses and Disclosures of Protected Health Information”. Access is limited to activities as required by job duties and responsibilities. Personnel will disclose immediately to his/her supervisor any potential conflict-of- interest pertaining to clients assigned. No PHI record will be accessible to the case manager that creates a potential conflict-of-interest as defined in applicable laws and regulations and GuidingPoint policies and procedures (e.g. records of friends, neighbors and relatives without the proper authorization of the client to access those records).
POSITION SUMMARY: The Resident Care Coordinator provides essential support and advocacy to residents of housing facilities who are living with mental illness. Operating under the guidance of an independently licensed clinical supervisor, the coordinator works directly within housing units to ensure residents receive the care and assistance they need. This role involves maintaining a consistent presence in the facility, offering professional support services, and fostering a safe, supportive living environment. Responsibilities include delivering clinical services in accordance with the standards set by the Ohio Department of Mental Health and Addiction Services (OMHAS) and addressing the unique needs of residents through personalized, on-site care and coordination.
- This position provides an array of services delivered to the mental health or substance use disorder population in accordance with applicable standards of care for Community Psychiatric Supportive Treatment (CPST); Therapeutic Behavioral Service (TBS) and Psychosocial Rehabilitation (PSR).
- This position is expected to work with a multidisciplinary clinical and non-clinical professionals and community agency personnel involved with the client and/or client’s family such as the Judicial System, ODJFS, school, etc. The goal is to address the (a) client’s needs and (b) collaborate regarding the client’s service outcomes.
- This position coordinates and monitors medically necessary services to promote continuity of care to (a) minimize the incidence and prevalence of symptoms of mental illness; (b) supports the client to remove barriers to recovery and promote successful tenure in the community; (c) facilitate community integration.

- Prepares for and attends all required meetings, workshops, and seminars. Participates in and completes ongoing training on support services to enhance skill level and promote agency mission and goals.
- Demonstrates commitment to community support program philosophy and standards, fostering environments conducive to recovery for persons served. Possesses working knowledge of available community resources and effectively manages caseloads. Capable of working with clients with diverse needs, including specialized services for the elderly, aggressive, dependent care, dually diagnosed, etc. Maintains agency standards of productivity and delivers expected client outcomes while effectively managing time and communicating with service providers and individuals at various socio-economic levels.
- Represents GuidingPoint LLC in both public and private settings during service delivery, maintaining professionalism and upholding the organization's values and reputation. Serves as a visible and accessible representative of GuidingPoint LLC within the community, fostering positive relationships with stakeholders and promoting collaboration and partnerships. Demonstrates thorough knowledge of GuidingPoint LLC's mission, services, and policies, effectively communicating with persons served, personnel, and stakeholders.
ESSENTIAL DUTIES AND RESPONSIBILITIES include the following. Other duties may be assigned.
Community Psychiatric Supportive Treatment (CPST): Provides specific, measurable, and individualized services to each person served. Services should be focused on the individual's ability to succeed in the community; to identify and access needed services; and to show improvement in school, work and family and integration and contributions within the community. Activities of the CPST service shall consist of one or more of the following
- Ongoing assessment of needs
- Assistance in achieving personal independence in managing basic needs as identified by the individual and/or parent or guardian
- Facilitation of further development of daily living skills, if identified by the individual and/or parent or guardian
- Coordination of the ISP, including: Services identified in the ISP; Assistance with accessing natural support systems in the community; and Linkages to formal community service/systems
- Symptom monitoring
- Coordination and/or assistance in crisis management and stabilization as needed
- Advocacy and outreach
- As appropriate to the care provided to individuals, and when appropriate, to the family, education and training specific to the individual's assessed needs, abilities and readiness to learn
- Mental health interventions that address symptoms, behaviors, thought processes, etc., that assist an individual in eliminating barriers to seeking or maintaining education and employment
- Activities that increase the individual's capacity to positively impact his/her own environment.
Therapeutic behavioral services (TBS): Provides individualized support or care coordination of healthcare, behavioral healthcare, and non-healthcare services. TBS may involve collateral contacts and may be delivered in all settings that meet the needs of the individual. Service activities include, but are not limited to the following
- Consultation with a licensed practitioner or an eligible provider pursuant to paragraph (C) of this rule, to assist with the individual's needs and service planning for individualized supports or care coordination of healthcare, behavioral healthcare, and non-healthcare services and development of a treatment plan
- Referral and linkage to other healthcare, behavioral healthcare, and non-healthcare services to avoid more restrictive levels of treatment.
- Interventions using evidence-based techniques. Developing and providing solution focused interventions and emotional and behavioral management drawn from evidence-based psychotherapeutic treatments.
- Identification of strategies or treatment options. Assisting the individual and family members or other collaterals to identify strategies or treatment options associated with the individual's mental illness.
- Treatment planning. Participating in and utilizing strengths based treatments/planning which may include assisting the individual and family members or other collaterals with identifying strengths and needs.
- Restoration of social skills. Rehabilitation and support with the restoration of social and interpersonal skills to increase community tenure, enhance personal relationships, establish support networks, increase community awareness, develop coping strategies, and promote effective functioning in the individual's social environment including home, work and school.
- Restoration of daily functioning. Assisting the individual to restore daily functioning specific to managing their own home including managing their money, medications, and using community resources and other self-care requirements
- Crisis prevention and amelioration. Assisting the individual with effectively responding to or avoiding identified precursors or triggers that would risk their remaining in a community setting or that result in functional impairments. Activities may include, but not be limited to, assisting the individual and family members or other collaterals with identifying a potential psychiatric or personal crisis, developing a crisis management plan, and/or, as appropriate, seeking other support to restore stability and functioning.
MINIMUM QUALIFICATIONS: Experience required in the delivery of culturally relevant and specific services. Must be available to see clients during day time hours, day treatment programming and critical times, which may include evenings and weekends. Valid Ohio driver’s license, a working vehicle, and current automobile insurance which meets state minimum requirements.
- Bachelor’s level preferred. GuidingPoint will consider a high-school graduate with 3-years of related training and experience.
- Able to participate in First Aid, CPR, Verbal/Physical De-escalation; and GuidingPoint Required professional training and development. Person must be mobile to assist clients in the natural environment.
ESSENTIAL JOB REQUIREMENTS: The following are requirements of this job and must be met and maintained at all times:
- Possess a working knowledge of navigating electronic medical records and general computer use/functions.
- Ability to pass criminal BCI and FBI background security screenings as required by regulations/ laws related to: a) performing the essential duties of this job; b) working in and accessing partner institutions such as the criminal justice facilities, school, residential facilities, etc., c) working in and accessing a MTHSS institution such as a childcare center, residential center, or senior center, etc.
- Possession of all valid professional licenses required by regulations/laws and accreditation related to performing the essential duties of the job.
- Possession of a valid Standard Care Agreement if applicable to this job.
- Possession of an automobile drivers insurance policy that meets state minimum requirements
- Possession of an automobile with valid plates which meet state requirements
