WHAT YOU WILL DO :
The System Navigator is a non-exempt position responsible for providing assessments to children and families in the home, school, and community settings, providing linkage and referrals to services and supports, both formal and informal, appropriate to the family’s needs. This position would require driving to see clients in Pinellas County several times a week.
- Complete client screening in a timely and effective manner - identify strengths, limitations, risk factors, and needs, and to refer and coordinate the provision of concrete services to children and their families
- Complete intake, including Family Self-Sufficiency Matrix, and Family Support Plan with the family. Provide ongoing evaluation and monitoring of the Family Support Plan
- Provide information, referral and follow-up for appropriate community services, such as counseling, psychiatric services, financial, employment, parenting, housing, and other services as deemed appropriate to meet family needs
- Provide effective role modeling and mentoring to families in a family-centered, youth-guided approach, congruent with the desired outcomes addressed in the Family Support Plan
- Maintain face-to-face and / or telephone contact with each assigned family, and with provider agencies servicing families to facilitate effective collaboration for appropriate services
- Work cooperatively with Program Director, Lead System Navigator, and Juvenile Welfare Board Utilization Management to determine family need and eligibility for financial assistance. Use Juvenile Welfare Board (JWB) Service Point in order to submit service requests for financial assistance to eligible families
- Perform all other duties as assigned
