We are looking for a Direct Care Worker to join our team. The successful candidate will be responsible for providing support and services to individuals with disabilities and complex needs within a residential setting. This is a great opportunity for someone who is looking to move into a leadership role within their field, as the successful candidate will be trained in the skills necessary to become a Direct Care Supervisor.
Responsibilities:
- Handle a caseload of clients.
- Conduct home visits with patients.
- Communicate with relevant parties (e.g. family members and medical professionals).
- Care for individuals with special needs.
- Conduct individual therapy sessions.
- Assist individuals with personal care and oriented towards self-sufficiency (e.g. bathing, grooming, dressing, eating).
- Enforce medication schedules.
- Help make safe and healthy food choices for individuals and families.
- Make referrals for social services (e.g. housing, financial assistance, etc.).
- Establish life-goals for individuals in need of guidance.
The position requires a High School Diploma or Equivalent with a minimum of one (1) year experience working in human services or a related field. The position requires a reasonable combination in of the following skills: good written/verbal interpersonal skills, the ability to develop and maintain positive relationships, documentation, and the ability to represent the Agency in a highly professional manner, with the ability to maintain the highest level of confidentiality. Applicant must pass Criminal Record Background Check and LA Motor Vehicle Record Check, have a valid Driver's License, current auto liability insurance, and have access to a reliable vehicle in a safe operating condition with current inspection stickers.
