Gulfside Healthcare Services is expanding our Hospice Service Line for services in Hillsborough County. We are posting this unique opportunity to come grow with Gulfside in our new Hillsborough County operations. We estimate the start date of this position to be mid-end of February with additional positions to be added as we grow. Reporting to the Manager of Patient Access, the Patient Access Specialist is committed to providing the highest quality of care as identified in the GHS Mission. The responsibilities of this position are to ensure a seamless coordination of patient care from initial referral contact through the admission process. This position will also act as the primary in‐office customer service contact in the Patient Access Center via telephone with referrals for patient care from physicians, hospitals, the community, and all other referral sources.
EDUCATION AND QUALIFICATIONS:
- HS Diploma Required
- CNA experience preferred
- Minimum of one (1) years of experience in a healthcare setting required
- Clinical sales experience preferred
- Bilingual a plus
ESSENTIAL JOB RESPONSIBILITIES:
- Ensures accurate and timely entry of patient’s referrals in all business lines.
- Acts in a professional and responsible manner within and outside the company; contributes to a positive image.
- Receives all incoming referrals and requests supporting documentation to facilitate the referral-to ‐ admission process.
- Initiates, updates, and completes assigned patient referrals by documenting patient information in the electronic health record.
- Referrals to be sent to Care Coordinator or covering staff within 15 minutes of referral receipt.
- Ensures consents are accurate and completed according to GHS standards, Florida State and Federal Regulations.
- Maintains close contact with Admission Nurses and Hospital Nurse Care Coordinators, informing of all new referrals and follows the progress of existing referrals.
- Responsible for ensuring travel contract patients are entered appropriately.
- Maintains records of Admission and Referral Logs and updates them daily.
- Accurately documents referrals not accepted for admission, the reason, and plan for follow up.
- Maintains a professional relationship and serves as a liaison with other disciplines.
- Possesses the ability to multi‐task and exhibits adaptability and flexibility in maintaining compliance in correlation with GHS standards.
- Ensures all documentation is accurate and is compliant with GHS standards, Florida State and Federal Regulations.
- Enters daily updates from Nurse Care Coordinators and Admission RNs into system for reporting purposes.
- Enter admissions and scatter beds when admitted ensuring admission report is up to date.
- Assists with entering of Home Health referrals as needed.
- Assists with entering palliative referrals as needed.
- Rotating holiday coverage as assigned, minimum of two holiday shifts per year.
- Ensures professional standards of Hospice nursing practice are maintained.
- Serves as a professional resource by assisting in the mentoring of new staff when requested.
SKILLS AND COMPETENCIES:
- Maintains up to date skill sets as required to care for patients
- Possesses ability to multi-task and adjust to shifting priorities, ambiguity, and rapid change; exhibits adaptability and flexibility in meeting GHS objectives
- Demonstrates excellent oral communication skills
- Ensures compliance with all patient care and documentation standards and regulations
- Assists in the implementation and maintenance of the policies and procedures of the Business Development Department; adheres to and assists in the evaluation of the same
- Assists in maintaining federal and state regulatory compliance
- Proficient use and entering information into Electronic Medical Records preferred.
