Scheduled Weekly Hours:
32Gundersen is now hiring a part time Social Worker for ambulatory care!
What you will work:
.8 FTE = 64 Hours every 2 weeks
Days, PM's, Nights, Rotating weekends and holidays
Location: Emergency Services La Crosse
What you will do:
Ambulatory social workers provide a variety of services to our patients in an outpatient setting including pre-operative discharge planning, care coordination, care navigation, resource connections, supportive listening, ACP completion and long-term care planning
Although based in La Crosse, this position assists with service needs in all our WI clinics and other areas as needs arise

What you will need:
Bachelor’s degree in Social Work or in a related field
WI Social Worker license
It is desired candidates have 1 year work experience in social services in a hospital, medical clinic, county social service agency or nursing home.
