Benefits
Specialist will offer the community resources, referrals and service coordination for grant funded programs such as Regional Care Coordination Organization, Healthy Communities, Handicapped Children’s Program and other care coordination services. Case Management for enrolled members will be offered and coordinated when appropriate. It is the intent for the specialist to help consumers navigate the systems of services currently being provided in Gunnison County and to assist access to care throughout the state
Pay Rate
The hourly range $25.01 to $28.48 depending on experience, plus partial benefits.
This position is currently supported by grant funding, which allows us to advance the county's strategic initiatives. Employment beyond the current grant period is dependent on funding availability.

Requirements
- Must possess a valid Colorado driver’s license.
- Must complete ICS 100 and ICS 700 within the first year of employment.
- Bachelor’s degree in one of the human behavioral science fields.
- Three (3) years of progressively responsible experience in health-related field.
- Any equivalent combination of education and experience that the County deems appropriate.
