What are the responsibilities and job description for the Peer Support Specialist/Patient Liaison position at Gunnison Valley Health?
The Peer Specialist/Patient Support Liaison is a person with lived mental health experience who uses their recovery experiences as a means to help others diagnosed with a mental illness find their own recovery process. The peer specialist is part of a multi-disciplinary team that participates in the planning and organizing of individual and group activities that promotes a range of self-help skills.
Position Qualifications
Education: High School or equivalent.
Experience: Advanced computer and keyboarding skills. (Must be able to operate basic functions in Microsoft Office (Outlook, Word) and several EHR systems.) Successful completion of Peer Specialist Training and Peer Specialist Internship Program or a willingness to obtain this training. Experience working with consumers and/or family members of a consumer of mental health service is a plus. Experience as a family advocate is a plus.
Licenses/Certification: Must have a valid Colorado driver’s license within thirty days of hire. Must be trained in transfers and first aid upon hire.

Responsibilities:
We are committed to improving the health of our community by delivering exceptional care and services. As part of the team in the ED, you will:
- Provides crisis intervention, evaluation and case management services to clients accessing the emergency services system.
- Assesses patient or family questions and concerns and develops a plan to address needs. Communicates information to appropriate persons in appropriate time frames to meet patient and family questions, concerns or expectations.
- Consults and collaborates with colleagues, peer, supervisors and other health care providers in a professional manner to improve the quality, effectiveness and efficiency of patient care.
- Facilitates psychosocial rehabilitation learning processes to support client recovery by:
- Assessing functionality
- Assessing readiness
- Goal setting

Directing and teaching skills that include illness self-management, social and interpersonal effectiveness, independent community living competencies and job readiness.
Physical Requirements:
Occasionally (1-33%) - Standing, walking, reaching, reach across midline, handling, crouching/squatting, stooping, stairs, lifting (0-20 lbs), carrying (0-20 lbs), pushing/pulling (20-50 lbs)
Frequently (34-66%) - Sitting, pinching
Continuously (37-100%) - Must be able to see with corrective eyewear, must be able to hear clearly with assistance
Compensation: $18.00 - $22.05/hr, depending on experience.
