H.E.A.R.T.S. was established ten years ago as a non-profit organization in Durham, NC. We provide programming and support to pregnant and parenting adolescents, age 13-22, to help them with life skills training in the following areas : education, self-development, reproductive health, family development and financial awareness. The Advocacy Coach will provide direct and personalized one-on-one case manager support to participants through weekly check-ins and monthly meetings. The Advocacy Coach helps to connect participants to support programs and services in the community. In addition, the Advocacy Coach helps to organize and run monthly Group Sessions and Creative Learning Experiences. An ideal Advocacy Coach for H.E.A.R.T.S. is hard-working, detail-oriented, committed to mission of H.E.A.R.T.S., is an empowering coach, patient, and understanding. Job Responsibilities : Interview and orient new participants to the program Help participants to define his / her needs and goals Assist with securing necessary emergency support services for participants Plan, organize and implement theory-based life skills trainingCapable of building a program from the ground up Coordinate one-on-one case management meetings Assist with group session planning and facilitation Implement home visits with all participants on the caseloadRefer participants to appropriate community resources Monitor participants attendance, goal achievements and needs Advocate and Parent Educator for participantsMaintain participants records, case reports and data files Document clearly all assistance and case notes Analyze data and outcomes Communicate effectively with Executive Director and team Stay up to date with professional development and trainings Become a Parent Educator - PAT CertifiedParticipate in professional organizations Requirements : Bachelors degree in social work from an accredited school of social work OR Bachelors degree in a human services field from an accredited college or university and one year directly related experience OR Bachelors degree from an accredited college or university and two years directly related experience (experience in the areas of case management, assessment, referral, supportive counseling, intervention, and goal planning) Capable of building a program from the ground up Parent Educator - PAT Certified preferred by not required.
