The Home Healthcare Liaison will play a crucial role in representing our organization and performing initial introductions to all service lines and products. This individual will be responsible for identifying opportunities for cross-selling and offering services necessary to meet patient needs.
The ideal candidate will possess strong customer service skills and be proficient in Microsoft Office Suite. A minimum of one year of customer service experience is required, as well as a high school diploma or equivalent. Homecare experience, sales experience, and two to four years of healthcare experience are highly desirable.
Key Responsibilities
- Represent the organization and perform initial introductions to all service lines and products.
- Identify opportunities for cross-selling and offer services necessary to meet patient needs.
- Gather specific patient information from charts and computer systems and forward to Central Intake for data entry.
- Provide information, answer questions, deliver collaterals to educate and promote the organization.
- Develop ongoing relationships with case managers, social workers, physicians, and staff members through demonstration of optimum customer service skills.
- Build relationships with referral sources to ensure seamless services for patients.
- Assure communication with operations is timely and appropriate.
- Utilize tracking system to follow up on pending cases.
- Act as liaison between patient and the organization team to welcome and explain product lines.
Customer Service Skills
The Home Healthcare Liaison must maintain exemplary customer service by providing understanding and responding to the needs of patients and families. The ability to lift a minimum of 10 pounds, push and pull a minimum of 10 pounds, and stand a minimum of 4 hours a day is also required.
