Check out the best places to work link!https : / / www.greatplacetowork.com / certified-company / 7044353Job Description : As a Hospice Chaplain, you will deliver compassionate spiritual care to terminally ill patients and their families in their homes. You will assess their spiritual needs and provide counseling to address those needs, while also facilitating visits from local clergy, pastoral counselors, or other supportive individuals. Additionally, you will actively participate as a core member of the Interdisciplinary Group / Team (IDG / IDT).Education : Master of Divinity preferred.Experience : One year of experience as a Chaplain in a healthcare institution preferred.Skills : Working knowledge of community spiritual and other resources. Good interpersonal skills.Transportation : Reliable transportation. Current auto liability insurance and driver's license if personal vehicle is used for work.Environmental and Working Conditions : Works in patient homes and other healthcare facilities under various conditions. Occasional exposure to infectious diseases; ability to travel locally; may have some exposure to unpleasant weather.Physical and Mental Effort : Prolonged standing, walking, and sitting required. Requires working under some occasionally stressful conditions to meet deadlines and patient / family needs, must be able to make quick decisions; functions as a facilitator to meet patient / family spiritual needs.Essential Functions : Evaluation : Assess the Hospice patient and support system to identify spiritual needs as evidenced by the Plan of Care (POC), documentation in the clinical records, and IDG / IDT conference minutes. Provide support for the spiritual needs of patient / family.Attend IDG / IDT meetings and participate in the patient care planning process.
