Hamaspik Homecare is seeking a compassionate and dedicated Intake Coordinator to be the first point of contact for potential clients and their families.

Key Responsibilities : Serve as the initial point of contact for potential clients and their families, answering inquiries and providing information about our services, policies, and procedures.
Conduct comprehensive assessments of clients’ needs, including physical, emotional, and social requirements, to determine the appropriate level of care and support.
Collaborate with clients, their families, and the nursing team to create personalized care plans that align with clients’ goals and needs.
Assist current clients in applying for additional services when eligible. Make referrals to social service agencies or community organizations for clients who need additional support beyond our scope.
Maintain accurate client files, including insurance information, consent forms, and other essential documentation. Foster strong relationships with clients, families, and stakeholders, ensuring empathy and effective communication at all times.
Ensure compliance with regulatory requirements and confidentiality standards. Participate in ongoing training and meetings to stay informed about industry trends, best practices, and regulatory updates.
Qualifications : At least 1 year of office experience, preferably in human services. Strong organizational skills and attention to detail.
