Hamburg Area Middle School Related Services Personnel are contracted through a service provider by the Hamburg Area School District and work directly with our students. If you are providing services through your employer to one or more students at HASD, please complete our brief intake form for proper submission of your background clearances and contact information.
If you work with students in more than one building, select the application / intake form for the building you most frequently visit. The intake form will ask you to specify all buildings you need access to. These preferences may be updated at any time by a building secretary.
Upon submission of your online intake form AND valid clearances (Act 151 Child Abuse / Act 34 PA Criminal History / FBI Unofficial Results) the following will occur :
- You will receive an automated email from "mailbot" asking you to complete our Act 24 Arrest / Conviction Certification form. Please look for this form (check SPAM) and execute electronically.
- Your documents will be verified by the applicable building secretary. You will receive an email approval or a request for more information.
- Once approved, you will be added to Frontline Central, our central repository for contracted staff.
- You will receive an email invitation to create an account through Frontline Central, the final step in our process. You must act on this request by creating your account. Invalid accounts will be terminated after 30 days.
- Frontline Central will notify you electronically 90 days before your clearances expire. You must provide the district with you updated clearances within that 90-day period to remain active.
All Related Services staff entering a building must first be approved by the appropriate District personnel and must have an account set up in our database, Frontline Central.
