Position Summary
The Outreach Navigator will support outreach and enrollment activities. The Navigator will attend and coordinate outreach events and provide one-on-one navigation assistance to enroll and renew eligible children and parents in available health care coverage.
General responsibilities
- Provide enrollment assistance (including but not limited to completing coverage applications, gathering the required documentation and troubleshooting the enrollment process) for uninsured children and adults to access health insurance.
- Develop and attend community events to promote insurance coverage options and the mission and health care services of the organization.
- Develop collaborative efforts with community organizations.
- Distribute outreach and health care materials to patients, community members, partner organizations and businesses to build coverage option awareness and HCHN brand awareness.
- Collaborate with local organizations to build awareness of coverage options, spur enrollment and build referral linkages.
- Assist in the development and implementation of organizational outreach and enrollment initiatives.
- Participate in various task forces or committees addressing program needs.
- Participates in health fairs, and other community projects with the goal of increasing awareness of our programs and services.
- Attend and successfully complete all required training programs; participate in ongoing conference calls, webinars, and other professional development opportunities.
- Accurately provide required reporting to track goal achievement and client satisfaction.
- Other duties as assigned.
Qualifications
Education and/or Experience
Required:
- Associate degree and a minimum of two (2) years of experience working with outreach programs.
Preferred:
- Bachelor’s Degree and a minimum of one (1) year previous experience working with outreach programs, program development, or public relations.
- Clinical skills and certification to perform basic clinical procedures.
OR
- An equivalent combination of education and experience sufficient to successfully perform the essential duties of the job as listed above.

Work Environment
Work is partially performed (20%) in an office environment. Eighty percent (80%) will be spent performing in-field outreach and enrollment assistance activities. Duties will require travel throughout the health organization’s service area. Travel expenses will be reimbursed.
