What are the responsibilities and job description for the SENIOR CASE MANAGER position at Hamilton Madison House?
Position Summary
Provide case management services for adults aged 60 and older living in New York City Manhattan’s Community District 3, which includes Chinatown, Lower East Side, East Village, Two Bridges. Determine client’s needs and service preferences through service assessments; working with each client and family members to develop an individualized care plan; identifying programs, services, and resources to meet care plan goals; and monitoring the care plan by maintaining contact with client/family and service providers, adapting the care plan as client needs change. This position offers flexible hours, including the availability of evenings.
Responsibilities
Program
- Conducts detailed client intakes and assessments
- Develops, implements, and monitors individualized care plans based on client needs and preferences
- Maintains up-to-date knowledge of and working relationships with community resources
- Provides benefits & entitlements screening and assistance applying for/receiving services
- Maintains up-to-date case records on all clients, including detailed case notes
- Assists with other program activities as assigned
- Plays a critical role in emergency preparedness, response, and recovery activities including maintaining contact with clients, and other services essential during emergency situations.
Community and Outreach and Partnerships/Collaborations
- Work with Older Adult Center Director and team to maintain and expand existing community partnerships by being active and visible in the community and working closely with other organizations.
- Participate in outreach activities as needed
Minimum Qualifications
- Master’s Degree required; MSW or LMSW preferred
- Socially perceptive, empathic, nonjudgmental and a good listener
- Minimum 2 years’ experience in social services
- Sensitivity to and understanding of the needs of older adults. Prior experience working with older adults desirable
- Demonstrated knowledge of social theories, practices, guidelines and core competencies of social work profession
- Highly organized and detail oriented; able to plan, manage and successfully complete multiple concurrent projects, frequently with short deadlines
- Ability to handle complex and difficult situations in a professional manner. Demonstrated ability to conduct oneself in a calm and professional demeanor when dealing with the public and/or with difficult situations
- Ability to develop collaborative relationships with professional staff and community groups
- Excellent verbal and written communication skills
- Excellent computer skills required including experience with data management systems, and willingness to learn other programs as required.
- Bilingual preferred; English/Chinese
Hours: Monday to Friday, 9:00AM to 5:00PM, 35 hours per week
Salary Range: $65,000 to $70,000 depending on qualifications and experience
