Your Role
As a Case Manager in our Jail Diversion program, you'll work directly with individuals who have mental health challenges, substance use issues, and developmental disabilities and are involved with the criminal justice system. Your role is critical in assessing needs, coordinating services, and facilitating recovery while reducing unnecessary incarceration.

You will conduct clinical assessments to determine the most appropriate referrals and help support successful diversion from jail when appropriate. For clients for whom diversion is not suitable, you'll play a key role in providing jail-based treatment services, including mental health education, counseling, goal setting, budgeting, and employment readiness. Once a client is released, you'll assist with follow-up care to ensure continued treatment and compliance with court mandates.

You'll collaborate closely with community partners, including the courts, public safety officials, and other service providers, to support reentry and recovery for each individual you serve. You'll also manage a small caseload, participate in clinical consultations, and document progress in accordance with agency policies and procedures.

What We're Looking For
To succeed in this role, you'll need a strong foundation in human services and a deep understanding of the complex needs of individuals with serious and persistent mental illness and co-occurring disorders. Familiarity with the criminal justice system is essential, as is the ability to build trusting relationships with clients, families, and partner agencies.

A thorough knowledge of the principles of social casework and the ability to apply supportive counseling techniques is critical. You should understand the behavioral characteristics and symptoms associated with various mental health and developmental conditions, as well as the challenges related to substance use. Knowledge of human development, the impact of trauma, and issues related to HIV/AIDS is also valuable.

Strong communication and organizational skills are essential. You must be able to evaluate client behavior, maintain accurate records, and prepare reports that document treatment progress and service coordination efforts. You'll also need to be familiar with confidentiality laws, including HIPAA and 42 CFR Part 2, and understand the services available through federal, state, and local programs.

Minimum Requirements
Applicants must hold a bachelor's degree in Human Services, Psychology, Social Work, Sociology, or Counseling and have at least one year of experience in case management. A valid driver's license is also required.
