Your role as a Case manager, Jail Diversion

As a Full-Time Case Manager in Jail Diversion at Hampton-Newport News Community Services Board, you will play a vital role in delivering direct services and coordinating comprehensive support for individuals with mental health challenges, substance use issues, and developmental disabilities who encounter the judicial system. Your responsibilities will include conducting clinical assessments to guide appropriate service referrals, ensuring successful diversion from incarceration when possible. In cases where diversion is unsuitable, you'll facilitate jail-based treatment services that encompass counseling, education on mental health, budgeting, employment, and goal setting. Your collaborative efforts will enhance client motivation and assist with eligibility for State and Federal support, while follow-up care post-release will ensure compliance with treatment and court mandates.

Working closely with clients facing serious mental illness and co-occurring disorders, you'll provide clinical consultation, participate in initial assessments, and manage a small caseload, ensuring that agency policies are upheld and supporting reentry needs effectively.

A thorough knowledge of the courts and the criminal justice system is essential, alongside expertise in interviewing and supportive counseling techniques. You should possess a strong understanding of social casework principles, community resources, and regional/state programs for substance-abusing adults. Familiarity with symptoms and characteristics related to the client population is crucial for effective service delivery.

Additionally, knowledge of human development and behavior, as well as understanding HIV infection and AIDS, will enhance your ability to support clients. Strong abilities in evaluating behavioral observations, building rapport with clients and families, and maintaining effective relationships with public and private agencies are imperative. Lastly, excellent communication skills and the capacity to document and report client progress will be key to ensuring successful case management.

Knowledge and skills required for the position are:

The ideal candidate will possess thorough knowledge of the courts and criminal justice system, as well as expertise in interviewing and supportive counseling principles and techniques. A strong foundation in social casework methods and a comprehensive understanding of community resources, regional and state programs for adults with substance use issues, and disabilities impacting the client population is essential. The candidate should have familiarity with the symptoms and common behavioral characteristics of these disabilities, as well as a deep understanding of human development and behavior. Knowledge of HIV infection and AIDS, including transmission, risk assessment, prevention, treatment, and disease progression, is required. Familiarity with federal, state, and local support services, client rights, and confidentiality laws, including HIPAA and 42 CFR Part 2, is necessary. The candidate should have extensive knowledge of the services needed for individuals with serious and persistent mental illness. Strong abilities in evaluating and interpreting behavioral observations, building rapport with clients and their families, and addressing a full spectrum of service needs are essential. Effective communication skills, the ability to prepare documentation, and proficiency in maintaining narrative and statistical records and reports are also required. Additionally, the candidate should be adept at establishing and maintaining effective working relationships with colleagues and representatives from various public and private agencies.

Requirements:

Bachelor's degree in Human Services (Social Work, Psychology, Sociology, or Counseling) with one (1) year experience in case management services.
