What does a Service Coordinator - Assertive Community Treatment do?
As a Service Coordinator in our Assertive Community Treatment (ACT) team, you will play a vital role in supporting individuals with severe and persistent mental illnesses, primarily schizophrenia. Your expertise will help provide crisis support and intensive outpatient services that empower clients to maintain stability in their communities, particularly after discharge from psychiatric facilities. You will engage in comprehensive service coordination, performing assessments, treatment planning, and advocating for clients as they navigate their recovery journey.
What matters most
To excel as a Service Coordinator within our Assertive Community Treatment (ACT) team, a Bachelor's degree in Human Services as Social Work, Psychology, or Counseling is essential. Candidates should possess at least one year of experience in case management or service coordination, demonstrating a solid foundation in addressing the needs of individuals with severe and persistent mental illness. Must be registered with the Board of Counseling to practice as a QMHP-A or register with the Board of Counseling to practice as a QMHP-A trainee within the first 30 days of employment.
The knowledge and skills required for the position are:
- Bachelor's degree in Human Services such as Social Work, Psychology, or Counseling. One (1) year experience in case management or service coordination services.
- Must be registered with the Board of Counseling to practice as a QMHP-A or register with the Board of Counseling to practice as a QMHP-A trainee within the first 30 days of employment
