POSITION TITLE: Program Director
PROGRAM: Mental Health Rehabilitation
REPORTS TO: Program Director, Executive Director
SUPERVISED BY: Executive Director, Medical Director, Program Director
RESPONSIBILITIES:
1. Assist in ensuring all treatment plans are developed and implemented.
2. Provide oversight and assess the coordination of all services for the LBHP recipient.
This includes but is not limited to the following:
a. Assurance of active recipient involvement in all aspects of care;
b. Coordinate and manage all services provided by the LBHP agency.
c. Access and coordination of services provided through non-LBHP agencies.
3. Manage and assist LMHP with all mental health staff that serve as a member of his/her recipient’s team.
4. Assist LMHP and coordinate written weekly evaluations on supervised new hires.
5. Assist LMHP and coordinate written monthly evaluations on supervised team members.
6. Assist LMHP and coordinate written annual competency evaluations on supervised team members.
7. Complete Quality Service contact evaluation monthly on all team members on caseload; follow up on discrepancies with recipient and team member.
8. Evaluate charts of 80 percent of caseloads quarterly.
9. Consult with physicians, nurses, other clinic personnel, and community agency personnel to develop a social diagnosis and plan of treatment or service and to interpret psycho-social factors related to illness, treatment, and continuity of care needs.
10. Assist LMHP by coordinating and providing assistance in an interdisciplinary or multidisciplinary team approach to diagnosis, plan, and provide treatment.
11. Assist in arranging for and coordinating delivery of services in the broader community to address continuity of care needs.
12. Provide the business office with obtaining all required personnel records.
13. Document services provided following the clinical standards.
14. Participate as a staff member, work unit, and department committee throughout programs and resource activities.
15. Enter progress notes in CheckpointEHR within 24 hours of seeing the recipient with proper documentation and appropriate signatures.
16. Print all completed progress notes weekly if needed.
17. Complete and submit all Concurrent Reports at least 10 days before the expiration of current Prior Authorizations.
18. Regulates proper usage of Prior Authorization units for all recipients.
19. Provide leadership and assistance to all employees.
20. Attend all staff meetings.
21. Attend meetings and trainings that will benefit our company as requested by the supervisor. (i.e. CheckpointEHR, Magellan, community involvement, etc.)
22. Return phone calls and emails promptly.
23. Follow all safety rules.
24. Obtain 20 training hours as required for a full-time Non-Licensed Mental Health Professional or 16 training hours, as required, for a part-time Licensed Mental Health Professional yearly.
25. Actively participate in all evaluations conducted by our company
26. Actively participate in crisis interventions.
27. Assist and or create an ITP for the client.
28. Update TP for the clients in CheckpointEHR.
29. Has a positive attitude that is conducive to a stress-free atmosphere.
30. Refrain from gossiping.
31. Assist the Executive Director and CEO when needed.
32. Must be able to travel if needed.
Minimum Qualifications: A Master's Professional with five (5) or more years of experience in Mental Health Rehabilitation or an MA in Counseling or Social Work.

Pay: $50,000.00 - $55,000.00 per year
