What are the responsibilities and job description for the RN, Population Health position at Hansen Family Hospital?
Our Clinics are looking to add a Health Coach/Population Health. This is a full time position working 40 hours per week. The Health Coach/Population Health will work collaboratively with physicians, providers, staff, other health care professionals, and community resources to provide care coordination across the health care continuum for targeted patients within the community. Is an integral member of the health care team who works to ensure safety, best practice and high quality standards of care are maintained across the continuum. Responsible for coordinating a wide range of self-management support and disease registry activities for the targeted patient population. The Health Coach must be an RN with an active license in Iowa. The selected candidate must be currently certified as a Health Coach or willing to obtain this certification.
