What are the responsibilities and job description for the Case Manager position at Harlem United?
The Case Manager is an integral part of the team providing case management services to tenants in order to promote wellness, recovery, rehabilitation and housing stability. Responsibilities are implemented within the framework of agency policy, professional ethics, legal scope of practice and regulatory guidelines.
Essential Job Functions
The following duties are mandatory requirements of the job :
- Collaborate with team to formulate Individual Service Plans for clients.
- Ensure completion of comprehensive intake, initial assessments of clients.
- Provide case management to clients as prescribed by the Service Plan and document client charts with an appropriate note for each encounter.
- Assist clients with completing paperwork needed for entitlements including the Veterans Administrations (VA) benefits.
- Implement range of mental health services including psycho-education, crisis intervention and referral to individual or group psychotherapy / counseling.
- Work closely with the team and clients to ensure optimal execution of Service Plan including focus on life skills and ADL.
- Participate in daily check-in meetings, weekly rounds, and case reviews; consult with team members as needed.
- Collaborate with the VA, as appropriate, as well as other providers to coordinate and oversee the overall health maintenance of clients.
- Conduct home visits and establish a work schedule in collaboration with the team to ensure clients are seen as needed.
- Escort clients to appointments pertaining to medical needs, mental health, benefits, housing, etc.
- Engage and refer clients to psychiatric and medical services, both through the VA and in the community.
- Work closely with the Clinical Director to ensure psycho social assessments are conducted and updated as needed.
- Screen clients for suicidal and homicidal risk on ongoing basis and document in client charts.
- Participate in case conferences, as needed.
- Participate in facilitating appropriate hospitalizations (medical and psychiatric), crisis interventions and referrals as needed.
- Be able to work a shift from Thursday thru Monday
- Ensure SEPS / LINC, or other subsidies are active and up to date
- Perform other related tasks as needed
