Your Responsibilities
Community Outreach & Partnership Building: Develop and maintain relationships with Medicaid-focused agencies, community organizations, and healthcare providers to promote our Medicaid services.
Medicaid Service Expertise: Act as a knowledgeable resource on Medicaid eligibility, benefits, and requirements, assisting clients and families in understanding and accessing Medicaid-funded home care services.
Educational Outreach: Lead informative sessions and presentations on Medicaid services and eligibility, educating potential clients, caregivers, and referral sources about available support options.
Referral Development: Cultivate partnerships with healthcare professionals, case managers, and social service agencies to generate consistent referrals for our Medicaid services.
Community Events: Represent Harmony At Home Senior Care at local events, health fairs, and senior programs, expanding our reach and building community connections.
Client Admissions & Support: Guide clients through the admissions process, focusing on Medicaid-eligible individuals and families, ensuring a smooth, empathetic onboarding experience.
Data and Documentation: Track community outreach activities, referrals, and Medicaid service data to analyze trends and refine strategies.
Compliance and Ethics: Maintain adherence to Medicaid regulations and uphold ethical standards in all interactions.
Continuous Learning: Stay updated on Medicaid and healthcare industry changes to provide accurate guidance.
What We’re Looking For
3 years of experience in healthcare, community outreach, or Medicaid services, particularly in a home health or senior care setting.
Expert knowledge of Medicaid services and proven ability to communicate effectively with clients and referral sources about Medicaid eligibility and benefits.
Relationship-building skills with healthcare providers, social workers, and community organizations.
Proactive and driven, with a focus on connecting clients with Medicaid services.
Independent and collaborative, able to work both autonomously and within a team.
Local travel required to attend events and meet with community partners; reliable transportation and valid driver’s license necessary.
