We are looking for an RN who understands the uniqueness of providing end of life care to hospice patients. This is a CALLING - not a job.
Our RN Case Managers are a part of an EXTRAORDINARY TEAM, which includes an LPN, CNA, Social Worker, and Spiritual Care Counselor for each patient. In addition, patients and families have volunteers available to them for additional support.
As a case manager, you will create a care plan to manage the individual patient’s pain needs, order medications, address concerns of patient and family, and provide education on end-of-life issues.
Qualifications
- RN License
- Managed care experience
- Patient assessment skills
- Experience in discharge planning
- Ability to develop care plans
- Strong communication skills
- Background in hospice care
- Familiarity with HIPAA regulations
