Description of Role:
HHMC & FRC is seeking full-time Family Practice Physicians to add to our medical care team. This role will be responsible for seeing patients independently, collaborating with mid-level providers/medical assistants, and seeing patients referred by other physicians. After completion of training (6 months), this role will see 16-20 pts/day (8hr day) or 22-25 pts/day (10hr day).
Essential Functions:
- Provide complete, comprehensive, family-oriented outpatient care for patients throughout the life cycle, including health examinations and treatment for medical conditions, and any problem detected.
- Manage acute and chronic illnesses, develop/coordinate a plan of care, and determine the need for case conference.
- Make appropriate referrals to both the internal disciplines of the clinic and/or external agencies for additional services required.
- Work with the Behavioral Health Specialist to provide continuity of care for the mental health population.
- Provide clinic consultation to mid-level providers as needed.
- Provide accurate, timely documentation in the patient’s medical record of patient contacts, case planning and plan of care.
- Other duties as assigned.
Qualifications:
- Valid California Medical License (Required)
- Valid DEA Certificate (Required)
- Board Eligible/Board Certified (Preferred)
- Valid BLS/ACLS Certification (Required)
- Updated immunizations for Hep B, Tdap, and COVID-19
- Overall knowledge of medical office patient care
- Knowledge of confidentiality issues involved with release of medical information
- Must have excellent interpersonal skills including ability to interact with staff and providers at other medical facilities
