Pay rate is related to recent hands on experience, LNA license status (not required), and shift.

POSITION SUMMARY: Responsible for providing personal care services, assistance, support, and supervision to residents.

REPORTS TO: Resident Care Coordinator

REPONSIBILITIES:

  • Adheres to and conveys a philosophy that supports the dignity, privacy, independence, choice, and individuality of residents.
  • Follows resident care plans in providing services to residents.
  • Do personal laundry for residents if so requested.
  • Strip and make residents’ beds.
  • Assist in preparing meals, serving meals, and bussing tables during mealtimes.
  • Daily light housekeeping.
  • Assist residents with activities of daily living, including self-management, monitoring, or supervision of medications, bathing, dressing, eating, toileting, hygiene, and mobility.
  • Assist residents with oxygen, ostomies, and other medical problems.
  • Encourage socialization among residents.
  • Assist with the activity program, conduct activities as directed by the Activities Director or assigned by the supervisor.
  • Encourage and/or remind residents to participate in activities. Promote activities enthusiastically to pique resident’s interest.
  • Chaperone activities outside the facility when requested.
  • Monitor the health, safety and well-being of residents.
  • Document information pertaining to changes in resident’s functional status, personal needs, appointments, etc.
  • Assist in keeping the environment safe for residents.
  • Report concerns about residents to supervisor and co-workers.
  • Respond to resident emergencies and arrange for appropriate medical attention.
  • Provide transportation services.
  • Identify supplies needed to perform tasks and report needs to supervisor.
  • Practice safety in working and with equipment, chemicals, tools and utensils.
  • Adhere to and carry out all policies and procedures.
  • Establish and maintain constructive work relationships with co-workers, residents, families, and visitors.
  • Maintain confidentiality of verbal and written information pertaining to resident, facility operations, and personnel.
  • Promote Harmony Homes in a positive manner.
  • Review the Staff Communication log on a daily basis for information about changes in resident service needs and/or health conditions.
  • Assists residents with medications and/or treatments as indicated in the medication book, as assigned and allowed by regulation.
  • Responds to calls from the emergency call system in a timely manner.
  • Performs all other duties as assigned.

EDUCATION AND QUALIFICATIONS:

Must have a demonstrated ability to effectively communicate verbally, with the ability to read, write and understand English. Is able to regularly lift up to 30 pounds, to occasionally lift more than 30 pounds, and to spend long periods of time walking/standing. Preference will be given to candidates with 1-2 years prior experience working with the elderly, preferably in a residential setting and to who hold a Licensed (or Certified) Nursing Assistant Certificate. Must be at least 18 years of age, with a high school degree or equivalent GED preferred.
