The HarmonyCares Medical Group is a leading provider of home-based primary care services. We are seeking an experienced Nurse Practitioner to join our team and provide high-quality, coordinated healthcare to patients in the comfort of their own homes.

Responsibilities:

• Provide comprehensive primary care services to patients in their homes
• Conduct thorough assessments and develop individualized care plans
• Collaborate with our interdisciplinary team to ensure seamless care coordination
• Communicate effectively with patients, families, and caregivers to ensure optimal outcomes

Qualifications:

• Current licensure as a NP in the state where the HarmonyCares Medical Group office is located
• Federal DEA license eligibility
• Geriatric training or experience preferred
• Active CPR/BLS certification
