This is an exciting opportunity to join HarmonyCares, one of the nation's largest home-based primary care practices.
We are seeking a highly skilled and compassionate Nurse Practitioner to provide high-quality, patient-centered care to our geriatric patients in their homes.
The ideal candidate will have experience in home health, geriatrics, hospice, and primary care, with a strong focus on addressing the unique needs of each patient.
Responsibilities:
- Provide primary care to high acuity patients in their homes Monday - Friday
- Collaborate with a Medical Assistant partner to deliver comprehensive care
- Conduct thorough assessments and develop personalized care plans
- Manage chronic conditions and coordinate with other healthcare providers as needed
Qualifications:
- Licensed as a Nurse Practitioner in the state where HarmonyCares Medical Group is located
- Federal DEA License/eligibility
- Geriatric training/experience preferred
- Active CPR/BLS Certification
