Brief Position Overview:
Under the supervision of the IMDT Manager, the Black Maternal Health (BMH) Cohort Manager will oversee a team of coordinated care professionals to achieve the goals of the Black Maternal Health of ACCESS Harris. The Cohort Manager will be responsible for leading and supervising the coordinated care team, training new staff members, providing logistical guidance and support, maintaining a working knowledge of and compliance with ACCESS Harris objectives and procedures, and coordinating with additional supervisors to deliver superior client care. The Cohort Manager will also oversee outreach efforts to increase access, communication, community engagement, and education for residents of Harris County.
Duties & Responsibilities:
- Monitors objectives and activities across multiple maternal and child health work groups, committees, or action teams, developing Key Performance Indicators tracked monthly. Oversee operations/activities for the cohort-specific interdepartmental multi-disciplinary team. Be the main point of contact for ACCESS Harris IMDT Manager, care coordination team, and safety net department appointed contacts.
- Responsible for workflow facilitation that increases productivity and efficiency to meet ACCESS Harris program initiatives. Evaluate, document, and continuously improve team performance. Provides oversight and direction to employees within the team by organizing, maintaining staffing levels, ensuring new employee training, performance management, etc.
- Maintains other organizational aspects of the overall program, including the inventory of supplies, incentives, materials, and other items needed for the success of the project.
- Regularly monitors and tracks capacity, process, and outcome measures, and incorporates quality improvement strategies to refine implementation efforts as needed and to ensure appropriate inputs are in place.
- Develops monthly progress reports, budget reports, and other documents that must be internally maintained and externally reported. Create consistent outreach plans to provide education and awareness of ACCESS Harris.
- Work with the Community Health and Wellness leadership to plan and implement strategies for project sustainability, including resource development, resource leveraging, capacity-building, and stakeholder engagement.
- Develops a shared measurement system to evaluate policy, system, and environmental changes as outcomes to achieve the maternal and child health program goals.
- Collaborates with multi-sector stakeholders (e.g., health care, education, academia, community development, etc.) to implement the Maternal and Child Health Program.
- Other duties as assigned, including special tasks involved in responding to an emergency event.

- Bachelor’s degree in Business Administration, Healthcare Administration, Public Health or related field
Experience:
- Two (2) years of experience in a supervisory capacity leading a team to reach challenging goals and objectives in the social service space
- Valid Driver’s License (Texas upon hire)

Full-Time | Grant-funded
40 Hours per Week
Must be able to work outside normal business hours (evenings, weekends) if needed.
May be required to work long and irregular work hours in circumstances of public health emergencies.
Work Environment:
This job operates in a professional office environment with occasional visits needed to private residential homes, clinics, and other community organizations.
This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.
Physical Demands:
This is a largely sedentary role; however, some duties would require the ability to lift up to 20 pounds and bending & standing for periods at a time.
Work Location:
Harris County Public Health (2223 W. Loop South, 77027) - Will relocate to Downtown Houston by the end of 2022.
Employment may be contingent on passing a drug screen and meeting other standards.
