The hospice navigator is a hospital-based representative of hospice care whose primary role is to provide effective placement of home hospice care for patients located throughout the hospital. The hospice navigator will work with hospital staff and the General Inpatient (GIP) team to quickly identify potential hospice patients and work to get those patients prepared for a successful transition to home-based hospice care. Functions as an expert in home-hospice capabilities and answers questions from providers, patients, and their families to ensure that the hospice program is well understood and that successful hospital to home transitions occur wherever possible so that patients benefit from hospice while simultaneously reducing acute-care burden.
Present home hospice care to patients and families located throughout the hospital to understand the hospice benefit and desire to enroll in home hospice care.
Evaluate GIP patients for the potential transition to home hospice care.
Be visible and available in the Emergency Department (ER) and inpatient units to build relationships with teams in those areas to build awareness of hospice care and the relationships to call the navigator in early to assess potential hospice eligibility.

- Help facilitate referral and transition of patients from hospital to home hospice
