As part of the person centered care model, this role will participate in the daily management and delivery of care of patients currently on Telemonitoring service with an emphasis on those with cardiac and respiratory needs. This role will ensure the delivery of outcome based care by utilizing metric-driven decisions. This role will collaborate with clinical team members to achieve exceptional outcomes. The Telemonitor Nurse will collaborate with clinical team members in the delivery of services, identification of those patients that would benefit from telemonitoring and participate in the development, maintenance and implementation of program goals and objectives.
Reviews and addresses “red alerts” reported by the monitoring company. Conducts virtual video visits per the plan of care and documents in electronic medical record, calls, case conference, clinical notes related to patient’s care and TM service.
Collaborates with Cardiac Staff of Bridge, Transplant, LVAD clinics to identify and review mutual patients with the emphasis on education and compliance to enhance positive patient outcomes and reduced rehospitalization rates.
Develops an interdisciplinary approach to client care with agency staff.
The Telemonitor Nurse will participate in IDT meetings for patients on telemonitoring to develop best plans for positive patient outcomes and patient satisfaction.
Assists the Telemonitor Program in achieving desired clinical and operational performance measures through a holistic approach and collaboration with clinical staff, physicians and clinics.
Follows trends in specialty area for cardiac/pulmonary disease management for home care industry; makes recommendations of treatment options for incorporation into the program with an emphasis on best practice for telemonitoring and virtual visits.
Participates in training, coaching of new and existing staff.
Actively participates in the improvement process known as H3W to identify TM service line needs and development
Performs other duties as assigned
Qualifications
Education:
Associates degree, diploma in nursing or Bachelor’s Degree in nursing Education
Bachelor’s Degree in nursing preferred
Minimum two years of experience in homecare
Strong written and verbal communication skills.
Knowledge, Skills and Ability Requirements:
Ability to effectively communicate at all levels within the organization and share knowledge, ideas and information.
Able to understand problem situations, solve problems and independently assess a wide variety of tasks in order to effectively take action to identify solutions to benefit the client/agency
Knowledge of relevant industry standards for Disease Management o with an emphasis on Heart Failure Heart and related diseases
Ability to balance financial, quality, people and customer expectation appropriate to business situation.
Intermediate to advanced MS Outlook, Word, Excel and PowerPoint, and Microsoft Project.
