About the Role:
The Skilled Nursing Team Lead will oversee the Resident Assessment Instrument (RAI) process and conduct assessments and care plan coordination for our residents. This is a critical role that requires a high level of expertise and attention to detail.
Responsibilities:
- Supervise and coordinate the RAI process, ensuring accuracy and timeliness
- Conduct regular assessments of residents, including physical assessment, interview, and chart review
- Develop and implement effective care plans in collaboration with caregivers
- Monitor and evaluate resident outcomes, making adjustments to care plans as needed
- Stay up-to-date with AANAC requirements and maintain knowledge of the RAI process and PPS
Qualifications:
- Registered Nurse (RN)
- AANC certification a plus; RAC-CT
- Experience as an MDS Nurse
