About the Role:
Haven Home Health us seeking a Home Health Intake Coordinator to join our Scottsdale office team. This position is responsible for managing the intake process for new home health referrals, ensuring a seamless transition for patients, and supporting the clinical team with accurate and timely information.
Key Responsibilities:
- Process incoming home health referrals, verifying insurance and eligibility
- Obtain necessary documentation and physician orders for new admissions
- Communicate with referral sources, patients, and caregivers to coordinate services
- Accurately enter and maintain patient information in the EMR system
- Work closely with clinical and office staff to ensure a smooth intake process
- Ensure compliance with Medicare, Medicaid, and private insurance guidelines
- Provide excellent customer service and maintain professional relationships with all stakeholders
Qualifications:
- Previous experience in home health intake, scheduling, or authorization coordination preferred
- Knowledge of Medicare, and private insurance guidelines
- Strong organizational and multitasking skills
- Proficiency with electronic medical records (EMR) systems (Kinnser/WellSky preferred)
- Excellent communication and customer service skills
- Ability to work in a fast-paced environment and collaborate with a team
