Position Title: RN Case Manager
Overview: The RN – Case Manager - provides full range of hospice nursing care to patients and families in care center settings. This position monitors patient's medical conditions and provides accurate reports to attending physician. They also coordinate case management of patient services and medical care and work in an interdisciplinary care setting to teach, assist, and support patient/family with the dying process. The RN Case Managers works assigned on call shifts. Due to business needs, may be required to work different shifts, teams and location.

You Have:
- Current Florida RN License required
- Graduate from an Accredited School of Nursing. Bachelor's Degree in Nursing preferred.
- Previous nursing experience preferred in oncology, hospice, palliative care, geriatrics, or home health care. Experience working with interdisciplinary team desired. Ability to work non-judgmentally with persons of varied cultural, religious and socioeconomic backgrounds and lifestyles.

Location: Orange Park
The Orange Park Field team is located at 745 Blanding Blvd next to our care center. The Orange Park Care Center is Haven’s fifth free-standing care center serving north Florida is a state-of-the-art facility that allows us to expand our services to better meet the end-of-life and palliative care needs of patients and families in the area.
