What are the responsibilities and job description for the Behavioral Health Navigator position at Hawaii Medical Service Association?
**Hybrid Work Environment - Must reside in Hawaii **
Pay Range: $47,500 - $88,000
Note: Individuals typically begin between the minimum to middle of the pay range
Screen identified HMSA behavioral health (BH) members into the appropriate program.
Minimum Qualifications
- Bachelor's degree in social services and one year of work experience conducting assessments in behavioral health
- Effective verbal and written communication skills
- Strong organizational skills.
- Knowledge of the appropriate protocol to be followed for a given BH diagnosis
- Basic working knowledge of Microsoft Office applications. Including but not limited to Word, Excel, and Outlook.
- Reliable transportation to attend in person (Face to Face) meetings
Duties and Responsibilities
- Assessment
- Provide telephonic screening of BH members using PHQ-9, GAD, and other CM-DM diagnostic tools to determine additional care support needs.
- Communication
- Communicates with HMSA members, physicians, case managers, and disease management team members.
- Coordinate with other colleagues to ensure continuity of care for members in assigned work queue.
- Triage
- After assessment, triage members to appropriate BH program
- Data Management and Documentation
- Complete and file forms and input data into appropriate HMSA systems.
- Education
- Provide navigation, referrals to community resources, and educational materials.
- Performs all other miscellaneous responsibilities and duties as assigned or directed
