What are the responsibilities and job description for the Care Coordinator position at HCAOA?
Care Coordinator is responsible for organizing and managing multiple aspects of our clients care giving services. Their duties include helping patients complete paperwork, communicating with our team about a patient's care plan and educating patients about resources and options for managing their health.
We are a 15 year established home care company that is focused on providing outstanding care to our clients in their homes.
Work from home, our office in Decatur and travel locally to Atlanta and Decatur locations to meet with clients and our caregivers.
Job requirements :
Must have home care, health care or related experience
Have clear background
Have reliable transportation
Job responsibilities :
Conduct intake and reassessments to determine client's needs and strengths
Create care plans, assist with schedules, and coordinate service
Facilitate communication between clients, family and staff
Provide instructions to caregivers and ensure adherence to standards of care
Complete paperwork and explain services we offer
