The Non-Medical Case Manager/Data Coordinator is vital to the Care Management team. This position ensures accurate client level intakes, data entry, participation in quality management and improvement projects, and supports all activities of the Care Management team. The Non-Medical Case Manager/Data Coordinator is responsible for collaboration with Northern Virginia Regional Commission for electronic database maintenance and the creation of standard and custom electronic database reports as requested by the Care Management Director, Assistant Director, and all other case managers. These activities are accomplished while adhering to all Virginia Department of Health Ryan White, The Health Resources and Services Administration HIV/AIDS Bureau (HRSA HAB) and the Department of Housing and Community Development (DHCD) Housing Opportunities for People with HIV/AIDS (HOPWA) requirements for client data and security. - Contributes to and participates in team Care Management culture.
- Maintains CAREWare 6 electronic database.
- Maintains Provide Enterprise® electronic database.
- Completes appropriate intake forms for clients and determination of eligibility for Ryan White Part B services.
- Completes non-medical case management service plan for clients.
- Coordinates appropriate referrals with community resources, ensuring Ryan White and HOPWA are payors of last resort.
- Maintains client charts meeting all grant requirements.
- Participates in Ryan White and HOPWA site visits, as determined by the Care Management Director and Assistant Director
- Verifies client and account data by reviewing, correcting, deleting, or re-entering data.
- Creates standard and custom reports, as needed for multiple programmatic requirements.
- Compares client data in differing electronic databases and identify missing data elements.
- Collaborates with Care Management Director to set, implement, and analyze yearly quality benchmarks.
- Identifies quality control processes to ensure criteria are always met.
- Maintains Care Management operations by following policies and procedures and reporting needed changes.
- Collates data from differing systems when client data is incomplete.
- Ensures required updates of software and train Care Management staff to implement necessary changes.
- Observes the dignity, confidentiality and privacy of patients and is aware of HIPAA guidelines.
- Participates in all required Ryan White, HOPWA, and database trainings.
- Provides educational resources to the Care Management staff, in collaboration with the Care Management Director, as appropriate.
- Collaborates with partner organizations including insurance companies, community resources, consistent with HIPAA guidelines to communicate patient care needs and to maintain continuity of care as needed or required.
- Assists in the orientation of new personnel.
- Performs other duties and responsibilities periodically, which the Care Management Director may deem necessary and appropriate.
- Management may modify, add, or remove essential job functions and assign other functions as necessary, or as changing organizational needs require.
- Must possess the ability to follow supervisor’s instructions and maintain regular attendance.

- High School Diploma or equivalent degree is required.
- Exceptional knowledge of Microsoft Office® applications.
- Experience maintaining a professional level of privacy in accordance with HIPAA and other relevant regulations.
- Efficient work management including planning, organizing, and prioritizing.
- Strong written and oral communication skills.
- Experience working with electronic databases and creating custom reports.
- Participation in continuing educational events regarding electronic database information entry, organization, and reporting requirements.
- Proficiency with computer technology and systems.
