Location: Windham and Windsor Counties
Health Care and Rehabilitation Services (HCRS) is looking for a compassionate and dedicated individual to join our team as a Shared Living Provider (SLP). In this rewarding role, you'll provide support to a 30-year-old female with moderate intellectual disabilities who will need a fully handicap accessible home. This individual would thrive best in a quiet and calm home with somebody who can provide structure and promote independence.
Key Responsibilities:
- Transportation to and from all locations with Client van.
- Support/oversight for medication administration
- Hygiene supports and reminders
- Foster a loving, comfortable home environment.
- Meal preparation.
- Support making and maintaining all medical appointments.
- Work collaboratively with a team of healthcare professionals, including case management, to ensure the best care and support.
About the Individual:
- Likes: This individual enjoys spending time with her mother and socializing. She is great around people and animals. She also enjoys watching tv and movies.
- Support Needs: This individual requires access to support as needed and is semi-independent. She requires transportation once a week to Ludlow to visit her mother. She needs full assistance when preparing meals but enjoys cooking and helping where she can. It is important for the home provider to continue to support her as she strives towards her own independence.

- Generous Compensation: A tax-free stipend of $30,000.00-$40,000.00 per year plus a monthly room and board payment of $826.65 directly from the individual.
